Some die from infections. The cultural belief driving it is that a woman's desire is something to be feared and controlled. Silvia's approach through EmpowerHer is not to arrive with a Western agenda but to educate, build trust, engage the elders, and find a new ritual that preserves cultural identity without physically harming girls.
